Instructions
The survey contains six different sections that ask general information about your company and specific information about your use of information technology and your experience with eBusiness applications. We recommend that you print a copy of the survey and identify the appropriate person within your company to respond to each section. Links to an Adobe Acrobat version of each section and the entire survey are provided below. ALL respondents should provide the individual and company identifying information requested in the first Section. Once this information has been provided, respondents may skip later sections by submitting a blank page. We estimate that it will take approximately 30 to 45 minutes to complete this survey. |

Participant Benefits
| Survey participants will benefit from this research project by learning more about the current state of eBusiness and its implementation in the semiconductor manufacturing industry and semiconductor equipment and materials industry.
Broad participation by U.S. and international members of the equipment and materials industry will
also help SEMI develop initiatives that support the eBusiness efforts of
its member firms. The UC Berkeley research team will tally and aggregate the completed surveys, and provide summary results to all survey participants. We will also provide a report detailing the major findings to all survey participants. |
